The Role of Ambulatory Care in Transitions: The North Carolina IMPACT ProjectDESCRIPTION
Between April 2012 to September 2013, nine North Carolina primary care practices participated in a care transitions learning collaborative through the North Carolina Area Health Education Center (AHEC) Regional Extension Center (REC) Program and Community Care of North Carolina (CCNC). Our presentation describes issues related to transitions in care, the innovative tools and use of technology utilized in this project, best practices related to transition care management, and opportunities for widespread quality improvement throughout our healthcare system.

Applying the Adult Education Learning Theory, Transformative Learning, to Promote Change and Impact Patient OutcomesDESCRIPTION
Transformative Learning Theory (TLT) is a systematic method for enabling change in adults and assisting primary care providers to adjust to the changing healthcare landscape. The essential steps of TLT will be presented with examples of NC AHEC REC using it to promote change and improve patient outcomes.
